In order to officially withdraw from NSU you must fill out and submit the Official Withdrawal Form or submit a written request which must include the following information:

• Social Security Number
• Full Name
• Current Address
• Phone Number
• E-mail address
• Semester and Year of Withdrawal
• Reason for Withdrawal
• Signature and Date (Signature is required because of Privacy Act regulations)
• Were you living on or off campus?
• Were you part-time or full-time?
• Did you receive financial aid this semester?
• Do you intend to return to college in the future?
• If so, would you consider NSU?
