• Automatic Deposit Transmittal Form (pdf)
• Computing and Telecommunications Action Form (pdf)
• Timekeeping/Leave System Set-Up Request  NEW for Banner 2012 (pdf) 
• A copy of the employee's Social Security Card is required to process Payroll.
