All classes must fit into your degree program.
Please email any changes after submitting Information Sheet.
DO NOT SUBMIT MULTIPLE SHEETS FOR SAME SEMESTER! Email office with all changes! Please ensure that the search bar at the top of the page is blank before filling out the form below and be sure to click the Submit Information Sheet rather than pressing the Enter key upon completion.

Statement of Understanding
- As an NSU student using VA benefits, it is my responsibility to report classes to the Veterans Services Office in a timely manner.
- As an NSU student using VA benefits, it is my responsibility to report any and all changes with enrollment, status, degree plan, benefits, and/or conflicts to the Veterans Services Office via email: firstname.lastname@example.org.
- As an NSU student using VA benefits, I certify that the all information given on the Information Sheet is correct to the best of my knowledge. If information changes during a semester, I will email the Veterans Services office at: email@example.com.
