Student Information Sheet is required every semester.  Meet with your academic advisor or follow the degree audit outlined for your major before enrolling and submitting this form.

  1. As an NSU student using VA benefits, it is my responsibility to report classes to the Veteran Services Office in a timely manner.
  2. As an NSU student using VA benefits, it is my responsibility to report any and all changes with enrollment, status, degree plan, benefits, and/or conflicts to the Veteran Services Office via email: veterans@nsuok.edu.
  3. As an NSU student using VA benefits, I certify that the all information given on the Information Sheet is correct to the best of my knowledge. If information changes during a semester, I will email the Veteran Services office at: veterans@nsuok.edu.
