Student Jazz Lab/CPA Request Form

Student Jazz Lab/CPA Request Form

This form is used for requesting events at the Jazz Lab or the Center for the Performing Arts.

There is a 14-day lead time for this request, if you need to schedule for less than 14 days out, you will need to contact the Music Facilities Manager and make those arrangements. 918-444-4603

**(For all Student requests there is an approval that is sent to Student Activities so that they can be sure the student organization's sponsor is aware of the request and the organization is in good standing.)
