Institutional Review Board Application

Submit an Application

To submit an application:
1. Download and complete the current IRB application (To complete the IRB Research Application, please download the Microsoft Word (.docx) document or Click Open with Google Docs > File > Make a Copy.). Consolidate all attachments into a single Microsoft Word document.

2. Send the application as an email attachment to irb@nsuok.edu. Copy all your co-investigators and/or your faculty sponsor (if applicable) on the email.

You will receive an automated notification of the receipt of your application. Within 48 hours, the IRB Chair will send a follow up email indicating the level of review assigned to your application and directions for digitally signing the application.

Applications may be submitted at any time. Exempt applications will receive approval within 48 hours. Expedited applications will be assigned to two IRB reviewers and will not need to be discussed in one of the board meetings. Applications needing full board review must be submitted by the deadlines listed in the meeting schedule in order to be reviewed at the next meeting.

Modifying an Application

When you would like to modify an existing application, you will do this in the following manner:

  1. Locate the originally approved application. If you are unable to locate your originally approved application, email the IRB Chair by email at irb@nsuok.edu and s/he will provide you with a copy of the originally approved application.
  2. Make modifications on the originally approved application and highlight your changes in an evident color. Resubmit your original application with the highlighted modifications. Copy all your co-investigators and/or your faculty sponsor (if applicable) on the email.

You will receive an automated notification of the receipt of your application. Within 48 hours, the IRB Chair will send a follow up email regarding the requested modifications.

Processing an Application

Applications are processed through the IRB email Monday through Friday during the regular fall and spring semesters excluding holidays and days which the university is not in session. Otherwise, applications are processed weekly.
